Request for network patch removal

Introduction

At NIKHEF, computer equipment is connected via a patch cable to an RJ45 outlet on the wall of offices and laboratory rooms. From there, UTP-5 cabling runs to a central patchpanel at two concentration points (in the H-building and the N-building), where the various ethernet switches are located.

A connection outlet is made active by establishing a patch between the corresponding outlet on the patchpanel and the appropriate ethernet switch port.

All patching is to be performed by authorized personnel only. Only then it is possible to keep the network administration up to date, which is necessary to keep track of all existing connections.

You are kindly requested not to plug, unplug, or move any computer equipment yourself.

Please submit the form below if there is a need to disconnect old machines.

Specify the current location of the machine involved. Enter both the room number and the number of the patch outlet.

Room numbers are of the form Bxyy, patch numbers are of the form Bx.zzz, in which B is the letter of the building (H or N), x is the one-digit floor number (0 - 3), yy is the two-digit room number on that floor, zzz is the three-digit patch number in that room.

Machine name

Enter the hostname of the machine involved, if it is an officially registered nikhef.nl machine. The name should have been stickered on the terminal screen and/or the machine cabinet.

Reason

This field should contain the reason for decommisioning this computer.

Requestor

This field should contain the e-mail address of the person who is submitting the current request. This is also the person who will be notified about the result of handling this network patch request by the system management.

Submit request

After the above fields have been filled in as completely as possible, the request can be submitted by clicking on the button below. The result of handling the request will be relayed to the requestor mentioned above.
