Request for network patch

Introduction

At Nikhef, computers and phones are connected via patch cables to RJ45 wall outlets in offices and laboratory rooms.

You are not allowed to plug or unplug equipment yourself, instead fill in and submit the form below if there is a need to connect or unconnect equipment to local network. Please specify room nummer (e.g. H133) and wall outlet (e.g. H1.031).

The CT helpdesk will handle your request as soon as possible.

Current location
Current room: Current wall outlet:

Specify the current location of the patch. Enter none in both fields in case there is no current patch.

New location
New room: New wall outlet:

Specify the new (desired) location of the patch. Enter none, in case the equipment involved is to be disconnected from the network.

Type of equipment

Indicate the type of equipment involved. This determines the proper segment of the network.

Host name

Enter the host name of the computer involved in case it is a Linux or Windows desktop managed by Nikhef. You can find the name labelled on the desktop and/or monitor.

In all other cases enter none.

Requestor

This field should contain the e-mail address of the person who is submitting the current request. This is also the person who will be notified about the result of handling this network patch request by the CT helpdesk.

Submit request

After the above fields have been filled in as completely as possible, the request can be submitted by clicking on the button below. The result of handling the request will be relayed to the requestor mentioned above.
